Mark your calendar, gather the necessary documentation, and pick up your vouchers.

If you’re unable to attend the distribution sites, don’t worry! Proxy forms are available for eligible seniors. A completed, signed form presented at voucher distribution allows someone else to pick up your vouchers on your behalf.

Incomplete or unsigned forms will not be accepted. To use a proxy, the proxy form must be:

  • Fully completed and signed by both the eligible senior and the designated proxy.
  • Presented at the time of voucher distribution at designated times and locations.
