YMCA Child Care Family Handbook

This handbook explains the philosophy, policies, and procedures of the Sewickley Valley YMCA Early Childhood Child Care Program. Families are encouraged to review and retain it for reference.

Forms

Financial Aid Application

When applying for financial assistance, families must complete the financial aid form and provide proof of income and documentation of expenses for verification. All personal information is kept confidential and reviewed only by YMCA professional staff.

Health Assessment: Child Health Report (55 PA Code §§3270.131, 3280.131, and 3290.131)

This state-required health assessment form must be completed by your child’s physician and submitted before attendance. It ensures your child’s health and immunization records are up to date.

Medication Permission Form: Medication Log (55 Pa. Code §3270.133; §3280.133; §3290.133)

Families must submit the medication permission form to authorize staff to administer any prescribed medication during program hours. Forms must be completed in full and signed by both the parent/guardian and the prescribing physician.
